Bear Paw Kids Management Authority
The Bear Paw Kids Management Authority (KMA) provides family driven, youth guided, and culturally competent coordination of services for youth with serious mental health issues and their families. Families are asked to join an Individual Care Coordination Team (ICCT) to determine the need for services and how services will be provided.
The Bear Paw KMA provides services for families with youth who meet the following criteria:
- Are 18 years old or younger (19 if still in high school);
- Have a need for coordination of services with more than one service provider, agency, or school;
- Have or be at risk for a serious mental health issue; and
- Are at risk for out-of-home placement or currently living in out-of-home placement.
Referrals
Referrals can be made by youth serving organizations or by families in need of care coordination.
For agencies, referral forms are available in PDF and Word form from the list below, or you can contact the KMA office. Referral forms should be completed only with the consent of the family and an Initial Release of Information form must accompany the referral form.
Families interested in self referrals can contact the KMA office to set up a meeting. All necessary paperwork will be completed with KMA staff and families working as a team.
